A good deal of my information on this topic comes from an active practitioner mailing list in trauma and critical care, certainly not a journal of record.  First, I'm not sure how to cite this at all. Second, I wouldn't do so without permission of the author. Third, while this is one of the articles I'm assigning to [[CZ: trauma medicine subgroup]], it was my thought to introduce the subgroup to the list when the time is ripe for recruiting.  Alternatively, I can go to a few individuals and ask permission, but I'm editing and paraphrasing anyway -- one distinguished contributor does so with his thumbs on a Blackberry, and even surgical dexterity can't necessarily produce correct spelling.
